Important note for ISLRR membersThe discount for ISLRR members for the Eurosight2000 conference in Veldhoven has been modified. Instead of 5%, ISLRR members may substract 10% from the prices for registration (not for hotel accomodation). However, to avoid any confusion, we kindly ask you to pay the registration fee as printed in the registration package and as can be found on our Web-site; the additional discount will be refunded when you register in Veldhoven. RegistrationThere is a limited number of participants, therefore places will be offered on first come, first serve base. Registered ISLRR members will have a discount on the conference fee. Acceptance of abstracts can only take place after receiving your registration payment.
Registration and feesPrint, complete and sign the registration form and hotel reservation form and return it to the Conference Secretariat. Registration is accepted once final payment has been received. You will receive a confirmation/invoice of registration and hotel reservation in the summer of 2000. Payment & additional information If you register for full registration, you will have a single room in the Golden Tulip Conference Hotel Koningshof or in the Golden Tulip Hotel Geldrop. There is a limited number of hotelrooms; reservations will be made on first come, first serve base. If you have a room in Golden Tulip Hotel Geldrop, transportation to and from the Conference Hotel Koningshof is arranged. Conference dinner The conference dinner will take place on Tuesday 5 September. You need to register seperately for this conference dinner in advance through the registration form. You will receive a conference dinnerticket together with your badge upon registration in Veldhoven. The dresscode for this dinner is casual. Payment The registration and hotel reservation form must be acccompanied by payment (certified cheque or credit card number) or proof that payment has been made by some other method (bank transfer). Payments have to be made in Dutch Guilders or in Euros and have to be received before 15 June 2000 (early registration). Payments made after 15 June 2000 are automatically late registration fee. Payment by credit card Fill out your credit card number, full name and address of the cardholder, expiry date and signature on the registration form. Only VISA or Euro/Mastercard is accepted. Make sure that you enclose a copy of both sides of your credit card. Payment by bank transfer Transfer the total amount to be paid to Rabobank Oosterhout-Lent, accountnumber 1279.123.20, in name of "Eurosight2000" mentioning "registration Eurosight2000". Please, state your full name and make sure your remittance is free of bankcharges. Bank charges will be deducted from you registration fee paid. Payment by certified cheque The cheque must be drawn on a Dutch bank, made out to the "Eurosight2000" Conference by mentioning "registration Eurosight2000" and must be sent (together with registration form) to the conference secretariat. The conference secretariat cannot be held responsible for cheques lost in mail. When sending Eurocheques, please note that the total amount per cheque cannot exceed Dfl. 300,00 and make sure that you fill out your personal number. Personal or postal cheques will not be accepted. Cancellations The date of receipt of cancellation (only accepted in writing) will be used to calculate cancellation charges.
If you have already paid and there will be a refundable amount, you will receive the refund after the conference.
