Daily Use Of Computorized Case Sheets

Synscentralen, the County ressource center for the blind and visually impaired, Storstrøms Amt, Denmark has compiled a 4-paged case sheed to be used as a part of daily work-instrument. Moreover the case sheet is on computer ( Access ), thus makes it possible measuring outcomes. The purpose is to help the user regaining a range of n ormal basic skills. Measuring is made by counting various steps of improvement.

These are related to the efforts, and explain the background.

The system includes the following areas, among others:

Everyday activities (ADL), assistive aids (ADL), tactile and visual markings in the home, communication aids, writing, lighting preferences (lux and Kelvin), Orientation & Mobility, orientation & mobility aids, optical devices (reading), CCTV, optical devices (distance).

Example: Activities of Daily Living, ADL

A severe visual impairment often makes it difficult to carry out ordinary activities around the house. Here is an example of the layout of the case sheet in this area:

The sheet records the client's subjective experience of his or her restrictions and abilities.

The assessment has four levels:

· blank: The Resource Center has not been involved in this issue

· 0 The Resource Center has been involved, and the status is positive (i.e., the person feels able to handle this area satisfactorily)

· - The Resource Center has provided assistance, but there has been no improvement

· + The Resource Center has provided assistance, and there has been improvement, i.e., the person now feels able to handle this area.
